NOTES:
at least 6 are necessary
must be present for at least 6 months to the degree that it is maladaptive and inconsistent with the person's developmental level
must be present prior to the age of 7
must occur in 2 or more settings

INATTENTION
1. fails to give close attention to details or makes careless mistakes
2. has difficulty sustaining attention in tasks or play activities
3. does not appear to listen when spoken to directly
4. does not follow through on instructions or fails to finish work
5. has difficulty organizing tasks and activities
6. avoids, dislikes or is reluctant to engage in tasks requiring sustained mental effort
7. loses things needed for activities
8. is easily distracted by extraneous stimuli
9. is forgetful in daily activities
HYPERACTIVITY (ADHD)
1. fidgets with with hands or feet or squirms in seat
2. has difficulty remaining seated when required to do so
3. inappropriately runs and climbs (excessively)
4. has difficulty playing quietly
5. appears driven or "on the go"
6. talks excessively
IMPULSIVITY
1. blurts out answers to questions before they have been completely asked
2. has difficulty awaiting turn
3. interrupts or intrudes on others
